Antidepressants

Antidepressants are a group of medications that can be used to treat a variety of disorders including depression, anxiety, obsessive compulsive disorder, eating disorders and certain personality disorders.


Selective Serotonin Reuptake Inhibitors (SSRIs)

  • Fluoxetine (Prozac, Sarafem);  Usual Daily Dosage Range: 20-80 mg
  • Sertraline (Zoloft); Usual Daily Dosage Range: 50-200 mg
  • Paroxetine (Paxil, Paxil CR); Usual Daily Dosage Range: 20-50 mg
  • Citalopram (Celexa); Usual Daily Dosage Range: 20-40 mg
  • Escitalopram (Lexapro); Usual Daily Dosage Range: 10-20 mg
  • Fluvoxamine (Luvox, Luvox CR); Usual Daily Dosage Range: 100-300 mg

Possible side effects include sexual dysfunction, gastrointestinal disturbances, insomnia, sedation, agitation, tremors, headache, dizziness, sweating, bruising, rare bleeding, rare seizures, rare activation of suicidal ideation and behavior, rare induction of mania, and rare serotonin syndrome.


Serotonin and Norepinephrine Reuptake Inhibitors (SNRIs)

  • Venlafaxine (Effexor, Effexor XR); Usual Daily Dosage Range: 75-225 mg
  • Duloxetine (Cymbalta); Usual Daily Dosage Range: 40-60 mg
  • Desvenlafaxine (Pristiq); Usual Daily Dosage Range: 50 mg
  • Milnacipram (Sarvella, Toledomin, Ixel); Usual Daily Dosage Range: 30-200 mg
  • Levomilnacipran ER (Fetzima); Usual Daily Dosage Range: 40-120 mg

Possible side effects  include headache, insomnia, sedation, anxiety, dizziness, nausea, vomiting, constipation, decreased appetite, sexual dysfunction, sweating, SIADH, hyponatremia, increase in blood pressure, rare seizures, rare activation of suicidal ideation and behavior, and rare serotonin syndrome.


Norepinephrine Dopamine Reuptake Inhibitor (NDRI)

  • Buproprion (Wellbutrin, Wellbutrin SR, Wellbutrin XL, Zyban, Aplenzin); Usual Daily Dosage Range: 150-450 mg

Possible side effects include dry mouth, constipation, nausea, weight loss, anorexia, myalgia, insomnia, dizziness, headache, agitation, anxiety, tremor, abdominal pain, tinnitus, sweating, rash, hypertension, rare seizures, hypomania, rare induction of mania, rare activation of suicidal ideation and behavior.


Serotonin Antagonist/Reuptake Inhibitors (SARIs)

  • Nefazodone (Dutonin); Usual Daily Dosage Range: 300-600 mg
  • Trazodone (Desyrel, Oleptro); Usual Daily Dosage Range: 150-600 mg

Possible side effects include nausea, dry mouth, constipation, dyspepsia, increased appetite, headache, dizziness, vision changes,  insomnia, agitation, confusion, memory impairment, sedation, syncope, hypotension, rare seizures, rare induction of mania, rare activation of suicidal ideation and behavior, and rare priapism.


Noradrenaline and Serotonergic Agent (NaSSA)

  • Mirtazapine (Remeron); Usual Daily Dosage Range: 15-45 mg

Possible side effects include dry mouth, constipation, increased appetite, weight gain, sedation, dizziness, abnormal dreams, confusion, flu-like symptoms (may indicate low white blood cell or granulocyte count), change in urinary function, hypotension, rare seizures, rare induction of mania, and rare activation of suicidal ideation and behavior.


Serotonin Partial Agonist Reuptake Inhibitor (SPARI)

  • Vilazodone (Viibryd); Usual Daily Dosage Range: 40 mg

Possible side effects include nausea, diarrhea, vomiting, insomnia, dizziness, bruising, rare bleeding, rare hyponatremia, sexual dysfunction, rare seizures, rare induction of mania and activation of suicidal ideation.


Other Antidepressant

  • Vortioxetine (Trintellix, Brintellix); Usual Daily Dosage Range: 10-20 mg

Possible side effects include nausea, constipation, diarrhea, dizziness, xerostomia, flatulence, abnormal dreams, pruritis, sexual dysfunction, vomiting, serotonin syndrome, abnormal bleeding, activation of mania or hypomania, angle closure glaucoma, hyponatremia in association with SIADH, rare activation of suicidal ideation and behavior, seizures, angioedema, depression exacerbation, and withdrawal symptoms with abrupt discontinuation.


Monoamine Oxidase Inhibitors (MAOIs)

  • Phenelzine (Nardil, Nardelzine); Usual Daily Dosage Range: 45-75 mg
  • Tranylcypromine (Parnate); Usual Daily Dosage Range: 30 mg
  • Selegiline (Eldepryl, EMSAM); Usual Daily Dosage Range: Oral 30-60 mg, Transdermal Patch 6 mg/24 hrs- 12/mg/24 hrs
  • Isocarboxazid (Marplan); Usual Daily Dosage Range: 40-60 mg

Possible side effects include dizziness, sedation, headache, sleep disturbances, fatigue, weakness, tremor, movement problems, blurred vision, increased sweating, constipation, dry mouth, nausea, change in appetite, weight gain, sexual dysfunction, orthostatic hypotension, hypertensive crisis, induction of mania, rare activation of suicidal ideation and behavior, seizures, and hepatotoxicity.


Tricyclic Antidepressants (TCAs)

  • Clomipramine (Anafranil); Usual Daily Dosage Range: 100-200 mg
  • Imipramine (TofranilI); Usual Daily Dosage Range: 50-150 mg
  • Desipramine (Norpramin); Usual Daily Dosage Range: 100-200 mg (Depression), 50-150 mg (Chronic Pain)
  • Amitriptyline (Elavil); Usual Daily Dosage Range: 50-150 mg
  • Nortriptyline (Pamelor); Usual Daily Dosage Range: 75-150 mg (Depression), 50-150 mg (Chronic Pain)
  • Doxepin (Adapin, Sinequan, Silenor); Usual Daily Dosage Range: 75-150 mg (Depression), 3-6 mg (Insomnia)
  • Trimipramine (Surmontil); Usual Daily Dosage Range: 50-150 mg 
  • Protriptyline (Vivactil, Triptil); Usual Daily Dosage Range: 15-40 mg
  • Amoxapine (Asendin); Usual Daily Dosage Range: 200-300 mg
  • Maprotiline* (Ludiomil); Usual Daily Dosage Range: 75-150 mg (Depression), 50-150 mg (Chronic Pain)

*Maprotiline is a tetracyclic

Possible side effects include blurred vision, constipation, urinary retention, increased appetite, dry mouth, nausea, diarrhea, heartburn, unusual taste in mouth, weight gain, fatigue, weakness, dizziness, sedation, headache, anxiety, nervousness, restlessness, sexual dysfunction, sweating, rash, itching, paralytic ileus, hyperthermia, lowered seizure threshold and rare seizures, orthostatic hypotension, sudden death, arrhythmias, tachycardia, QT prolongation, hepatic failure, extrapyramidal symptoms, increased intraocular pressure, rare induction of mania, and rare activation of suicidal ideation and behavior.
